	Reviewer Qualifications
	Board-certified physician Permedion performs our own Primary Credentials Verification and credentialing before the reviewer is allowed to look into your cases. Current unencumbered medical license (a physician who has full, unrestricted license-not on probation or supervision or regular reports to the State Board, etc.) Practicing physician/professional Licensed health care professional Expertise in clinical issue under review through actual practice experience
	Reviewer Training
	The applicant must have received medical necessity physician reviewer training by a Permedion physician, including:
	Phone review with Permedion physician Phone review with Permedion nurse Reviewer manual
	Biennial Recredentialing
	The terms of the reviewer contract require that any malpractice judgements be immediately reported to Permedion The terms of the reviewer contract require that Permedion be notified in the event of suspension of reviewer hospital privileges Questionnaire regarding any change in status of privileges, licensor, practice, location, workload Copy of new license registration card Copy of board certification renewal, if applicable
